Transplant Evaluation Care Coordinators Workflow - Nov 2024
The goal of this workshop was to investigate the workflow of Transplant Care Coordinators (Schedulers and Nurses) in the kidney transplant evaluation process. By engaging key stakeholders and team members, we aimed to uncover pain points, inefficiencies, and opportunities for improvement in the scheduling, coordination, and management of transplant patient referrals. This collaborative effort served as the foundation for designing a new custom system, the Transplant Evaluation Care Navigation program (TECN), built on Salesforce HealthCloud. The system aims to optimize care coordination, streamline administrative tasks, and ensure a seamless experience for both patients and healthcare providers throughout the pre-transplant phase.
