I served as the Lead UX Architect & Designer for multiple applications, including Tactical, Social Work Services, Transplant Referral & Evaluation, and PatientHub Rx.
CTH: Social Work Services - Transplant Referral
I worked on the full-stack design for Transplant Referral, from the initial concept to the fully implemented e-referral module. This tool reduced the time it took Social Workers to compile referral information from 4-6 hours to just 10-15 minutes. These changes led to a 25% increase in referral rates and a 17% boost in retention in just one year.
CTH: Tactical Care Team
The Tactical Teams—comprised of Nurse Managers, Social Work Managers, and Registered Dietitian Managers—are deployed to evaluate the strategic needs of underperforming clinics and partner with them to drive improvements in care delivery and patient outcomes. The tool we built empowers them to pinpoint gaps, prioritize interventions, and track progress collaboratively.

My Role:
As Lead UX Architect for the Tactical application, I helped shape the entire journey—from early whiteboard sketches and blueprinting to the live tool now used in clinics. My focus: turning complex quality data into actionable, user-friendly workflows that support treatment adherence and better outcomes.
CTH: Social Work Services
This application allows Social Workers to consolidate patient information, access nurses' notes, perform assessments, and submit Transplant Referrals. Additionally, we collaborated on integrating FindHelp.org for Social Determinants of Health assessments and resource connection. Before this integration, Social Workers and care coordinators kept personal resource lists either on paper or in digital formats, but they were not easily accessible or standardized. The process of connecting patients with resources was informal, often untracked, and lacked feedback on whether or not patients received care. Many community practices also lacked social work resources, leaving patients without proper care coordination.

My Role:

I guided the design strategy and conducted research to simplify the process for Social Workers, ensuring the platform was user-friendly and efficient.
CTH: Nutrition Services
The Nutrition Services Application was designed to streamline the workflow of (Registered Dietitians) in managing dialysis patient care. The app enables RDs to perform comprehensive assessments, create detailed care plans, and collaborate with other clinicians (such as nurses, Social Workers, etc.).
My primary focus was on the design of multiple assessment modules, including comprehensive nutrition assessments that evaluate a patient's nutritional status, track lab results, and provide personalized dietary recommendations to optimize health and manage kidney disease-related complications.
Additionally, I conducted research and designed the integration of FindHelp.org resources within the app. This integration empowers Dietitians to easily connect patients with valuable community resources that address social determinants of health, enhancing patient care and supporting better outcomes.
My Role:
I led the design process from research to initial wireframes and prototypes, ensuring a seamless user experience that meets the complex needs of Dietitians. The application was crafted to provide a user-friendly interface, making it easy for Dietitians to track patient data, collaborate with other clinicians, and access social resources, ultimately contributing to more efficient care delivery.
